Authorization for Information Sharing and Digital Data Transmission

Longevix, Inc. (“Longevix”) offers a variety of programs, services and products that are NOT currently governed by the Health Insurance Portability and Accountability Act and related implementing regulations (“HIPAA”) or by State privacy laws. By submitting information to Longevix, including personal identifying information, such as, without limitation, demographic details and contact information, you acknowledge and agree that the information may be utilized by Longevix for any variety of purposes, including, without limitation, for servicing your needs and promoting additional programs, services and products offered by or on behalf of Longevix and, potentially, by third parties. Such information may include health-related information not governed by HIPAA or State privacy law.

Longevix also operates in the capacity of HIPAA Business Associate of Yasha Magyar DO PC (the "Magyar Practice"). The Magyar Practice constitutes a Covered Entity under HIPAA. As such, HIPAA privacy and security protections apply to both the Magyar Practice and Longevix with respect to the DEXA scan, laboratory testing and any other applicable medical and healthcare related services offered by the Magyar Practice at the Longevix premises, unless an exception applies. The use, sharing, treatment and disclosure of your Protected Health Information (“PHI”) as defined by HIPAA is accordingly independently governed by the HIPAA Authorization with Yasha Magyar DO PC agreed to concurrently herewith.

Despite the foregoing, you acknowledge that if you provide or submit to Longevix or a third party, through any mechanism, any medical or healthcare related information unrelated to the performance of medical and related healthcare services rendered by the Magyar Practice in its capacity as Covered Entity, the use and disclosure of such information may NOT be subject to HIPAA and would instead be governed by this Authorization for Information Sharing and Digital Data Transmission with Longevix. You acknowledge that online forms and e-mail and digital communications may not be fully secure, and unauthorized access may be possible. While Longevix takes all reasonable precautions to protect your information, you acknowledge the inherent risks associated with electronic transmission and storage.
